Medicare Enrollment for Physicians and Other Part B Suppliers Fact Sheet — Revised
This fact sheet is designed to provide education on Medicare enrollment information and how to ensure physicians and other Part B suppliers are qualified and eligible to enroll in the Medicare Program. It includes information on how to enroll in the Medicare Program, how to report changes, and a list of resources.

Medicare Enrollment for Institutional Providers Fact Sheet — Revised
Hospices will need to submit their enrollment form to the Medicare carrier for their region.  A listing of the Medicare carriers and their regions is available online.

QPP and Palliative Care FAQs
At the bottom of page 12 of the FAQs, additional clarification is needed. If a provider is looking for data regarding how well he/she did under PQRS, the provider must create an EIDM (Enterprise Identity Management) account at https://portal.cms.gov.  It is through this portal that palliative care organizations and their physicians/NPPs can find their Quality and Resource Use Reports (QRUR).
